Desecrated tomb does not trigger when Scrapheap Scrounger returns from graveyard to the battlefield.

To add to this. It seems to not trigger at all for the first time a card effect; in my case Blood Operative; returns a creatures from the graveyard. The next time, not the the next turn but the next card effect for a graveyard return, ,triggers 4 bat tokens to appear.
At other times, again with Blood Operative, it triggers for one of it being placed on the battlefield. Then triggers again interrupting all the copies of BO returning to place the Bat tokens(i.e. 2 at a time like the DT card states). Or, it creates the Bat tokens after a while. Just got the token effect while I was checking my hand about 10-15 seconds after the last BO had been respawned. Closing the hand card screen to show them being generated.
