Not anymore, but my Nick Fury started out as a 0/9/0, and was a 2/13/3 until I finally stopped getting Demolition covers. This is about the only time I’d recommend buying one of the Heroes for Hire packs…if you’ve got a 4-star that’s 0/0/X, and really need that extra cover.
My Silver Surfer was 0/0/5 for ages. I forget how many black covers got sold before I got any other colors, but it was at least 3. He’s now 1/4/5 though, so making progress.
You have a character at 0/5/0, and it’s MORDO. I mean yeah, you had some bad luck with X23, but for lightning to strike again and hit MORDO?!?

On topic - I had Agent Venom at 0/5/0 for a while - he’s at 4/9/3 now, but at least the extra black covers were bunched together a bit so I could make a bit of extra ISO from them (apply 2 black covers, level to 183, then sell).
My Peggy got up to 8/1/0 before I finally got smart and stopped using her as my 4* Bonus Hero in hopes of picking up a blue Cover for her. Probably actually going to do the Heroes for Hire thing when her blue finally rolls around, because this is just getting too frustrating.
My X-23 was 5/5/0 since about two weeks after her release. I was lucky enough to earn four more green covers before I earned another one from the daily reward. At one point she was 9/8/0. I purchased her first purple cover from the heroes for hire sale a couple weeks ago. In one of the elite tokens I got a bonus green cover - I know its my fault for not actually picking bonus heroes. At that point I almost sold her.
The probability of drawing all the same colour cover for a character out of 5 covers total is (1/3)^5, which is about 0.4%.
If you’ve got 100 characters slots, the probability that none of them will produce 5 of the same colour cover on the first five pulls is (1 - 0.004)^100, or about 66%. That means that if we took three players with 100 character rosters, we’d expect one of them to see this condition.
It’s a bit more difficult to say how common it would be for the situation to persist. That’s going to depend strongly on which character it happens to: for a vaulted character or classic 5*, you could be stuck at 5 covers for a long time. For a character that is easier to acquire covers for, it is unlikely that it would persist for long: it grows increasingly unlikely to continue pulling covers of the same colour.
