This happened to me on multiple occasions.
During the RatC event 3.2.
AI casts any creature
Player casts Moon
…
AI casts Heart of Kiran
Player casts Claustrophobia (trapping HoK)
…
AI casts creature
Player casts frog
Heart of Kiran & Moon swap places.
This is repeatable so should be easier to find.
