In a game I played this morning, on several occasions, the cards in my hand sorted to an entirely different order than I had chosen. This was very annoying since my opponent was using Transgress the Mind. Instead of discarding the cheap but useless Titan’s Presence, it moved Unlicensed Disintegration up two spots and discarded it.

I noticed that too. It seems to happen after you are prompted to cast or not the spell. After you respond that spell is placed on top of your hand.
Had this same problem when I was playing against Leliana1 today with her first ability. The game was resorting my cards and she was discarding stuff I had in placed in a different order to keep her from discarding my already filled cards. Very annoying to say the least.
This has also been happening to me.
Exactly this. Declined spell and it jumped from third in my list to first over two zombies (both Fleshbag Marauder).
According to the bug list the devs are aware of this issue.
