Totally Lost did not return a creature to the top of it’s owner’s library?
I’m playing the final level of the “Duets of Mana - Tears and Memories” event (Level 8, “Mu Yanling, Sky Dancer”), and after casting a couple of Deploy the Gatewatches, I have Baral, Emrakul, The Promised End, and Totem-Guide Hartebeest in play.
Greg casts Totally Lost at my Emrakul. Here’s the board just after that happened:

The next card I drew should have been an Emrakul, but as you can see it was a Strategic Planning!
Nothing in my opponent’s graveyard or exile would seem to be relevant:
The 16 shield support that I own there is an Omniscience, and the event support doesn’t seem to be relevant either:
